PACEAPP requires that all child care providers participating in our program submit preliminary documentation before we can begin reimbursement for child care services. This documentation includes the information outlined below. We cannot begin issuing reimbursement for child care services until this information has been completed and returned to the Case Manager.
After a Case Manager has processed the provider's paperwork, the provider will have an active status in our program. We routinely update our provider files, and at times, we will request that providers send us current documentation. Current documentation may include: a recent copy of a certificate of liability insurance, an updated rate agreement or rate brochure, and a current W-9 form.
Below is a list of some of the forms required for new providers. If you are working on any of these forms and need assistance, please contact your case managers directly.
Licensed Center-Based Care and Family Child care Homes
- A copy of the child care license
- A current copy of liability insurance certificate
- A copy of rate brochure or price list
- Provider's policies
- A signed PACEAPP rate agreement
- A tax identification form (W-9 form)
- Provider agreements (State and Federal)
- Signed Fraud Policy receipt
- Copy of Family Child Care Provider's Social Security Card
- A copy of a valid California Driver's License or State issued I. D.
- A copy of provider's Social Security Card
- Exempt from TrustLine Health and Self-Certification or Proof of TrustLine registration/fingerprinting or LiveScan
- A signed PACEAPP rate agreement
- A tax identification form (W-9 form)
- Provider Agreements (State and Federal)
- Criminal Record Statement
- Non-licensed provider agreement and understanding
- Provider receipt and acknowledgment form
- Topics of discussion for parents and providers
